Remote Therapeutic Monitoring

Monitoring the ever-changing symptoms of your chronically ill patients can be time-consuming for your staff, so let the system automatically capture those activities so patients get better care and you get better reimbursements.

Extend your billable resources as they perform the care services your patients need

Under the RTM codes that CMS is continuing to refine in FY2023, your practice can qualify for additional reimbursements for many of those activities performed by non-physician care team members, such as respiratory therapists, physical therapists, and more.

Similar to RPM, RTM broadens the number and types of clinicians that can perform reimbursable care management services.  For example, for asthma patients, respiratory therapists can track data points like the use of an inhaler in regular and emergency situations, giving them insights into how the patient is trending. CPAP patients can report on things like utilization of their equipment and sleep schedules so care managers can better understand if the patient is able to maintain compliance with their therapy.

With ChronicCareIQ, you can extend the reach of your staff and the breadth of your services to more patients by allowing the system to automatically capture qualified care management-related activities without the added burden of documentation and compliance.

The Benefits of RTM

  • Get paid for more of the non-face-to-face work your team is already doing
  • Generate more revenue without adding staff
  • Extend the number of billable staff members in our practice
  • Reach more patients with a more productive staff
  • Prevent hospitalizations and disease progression
  • Improve the quality of care your current staff can deliver

“ChronicCareIQ gives our patients better access to us, and it gives us better awareness of their conditions. We've expanded it to all physicians, expanded our care to more patients and facilities, while providing a higher quality of care across the board.”

How ChronicCareIQ Enables RTM

As CMS continues to expand the CPT codes that incentivize physician practices to perform preventative care management, new types of qualified healthcare professionals can perform billable services.

ChronicCareIQ automatically captures all care management-related activities performed by your staff and documents those activities against the billable requirements so you can extend the reach of your services to more patients while maintaining your existing staff levels.

All different types of practices can bill for RTM once CMS approves the new codes that are expected to be in the FY2023 Physician Fee Schedule.
